CFBC Boiler explosion

We have faced boiler explosion in CFBC (Water cooled cyclone) of 85 TPH Boiler, as per our findings the same is due to high moisture and High VM Coal accumulation and then sponteenous combustion. Any such experience for remedial action or design change will be highly appreciated.
